Administrative Forms

These forms are mostly administrative in nature. They provide information ancillary to a petition or an application or they request information from BCIS (a.k.a INS).

AR-11 PDF 139KB	Change of Address Form	This form will become increasingly more important to the extent that the movement of nonimmigrants is tracked by BCIS (a.k.a INS).
G-14 PDF 4KB	Information Form	This form is used to provide additional identifying information so that the BCIS (a.k.a INS) may act upon or reply to a communication
G-28 PDF 111KB	Notice of Entry of Appearance as Attorney or Representative	This form notifies the BCIS (a.k.a INS) that an attorney or representative of a religious, charitable, social service or similar organization represents the petitioner and/or applicants.
G-325 PDF 191KB G-325A PDF 300KB G-325B PDF 16.2KB G-325C PDF 53KB	Biographic Information	These forms provide biographic and background information on persons involved in a petition or application.
G-639 PDF 105KB	Freedom of Information/Privacy Act Request	This is used to request copies of the file or other information maintained by BCIS (a.k.a INS) for any individual.
G-731 PDF 17KB	Inquiry About Status of I-551 Alien Registration Card	This form is used to inquire as to the status of a "green card" for an alien who has adjusted status and has not received a card.
