Department of State Forms Used by US Consulates

These are forms used to process immigrant and nonimmigrant visas at the US consulates. An applicant should check with the consulate to learn of the filing fees that are charged with each application as these vary from one country to another.

DS-156 PDF 225KB	Nonimmigrant Visa Application	This form is to be submitted to a US Consulate to apply for a nonimmigrant visa. Please note that form OF-156 is no longer used.
DS-157 PDF 123KB	Supplemental Nonimmigrant Visa Application	This is a form required of all male nonimmigrant visa applicants between the ages of 16 and 45. This must be submitted with Nonimmigrant Visa Application (DS-156).
DS-156E PDF 253KB	Nonimmigrant Treaty Trader/Investor Application	Use this form with the DS-156 Application when applying for E Visas.
DS-156K PDF 134KB	Nonimmigrant Fiancù(e) Application	Use this form with the DS-156 Application when applying for a visa as a finacù(e) of a US Citizen who is traveling to the US to marry.
DS-117 PDF 137KB	Application to Determine Returning Resident Status	This form is used by US residents to return to the US after a protracted stay abroad.
DS-3052 PDF 116KB	Nonimmigrant V Visa Application	Use this form with DS-156 when applying for a visa to join a petitioning relative in the US to await permanent residence processing.
DS-230 PDF 269KB	Application for Immigrant Visa and Alien Registration (Parts I and II)	These forms are used to process a permanent residence or Immigrant Visa at US Consulates.
DS-3032 PDF 87KB	Choice of Address and Agent	This form is used to instruct the Department of State on how to process your immigrant visa papers. It provides them with the name and address of the person who is authorized to receive your paperwork.
DS-3035 J-1 PDF 91KB	Visa Waiver Review Application	This form initiates the formal J-1 Visa waiver process. A fee of $230 must accompany the application.
