These are forms used in connection with the naturalization process. As with any Immigration Forms, you should read and follow the instructions very carefully. The key to success in naturalization is accurate and truthful information. When ever a doubt exists, the applicant should opt in favor of full disclosure. The BCIS (a.k.a INS) does not refund filing fees.

N-565 PDF 290KB	Application for Replacement Naturalization Citizenship Document	This form is used to apply for a replacement Declaration of Intention, Naturalization Certificate, Certificate of Citizenship, or Repatriation Certificate, or to apply for a special certificate of naturalization as a U.S. citizen to be recognized by a foreign country.	$155
